Description des services: This service provides a “no wrong door” to mental health services in assisting individuals by providing information to identify the problem and assisting them in accessing the appropriate services. This service does not preclude individuals accessing mental health and addiction services directly, but will assist those individuals seeking services or referring to them when they are uncertain of the problem and/or the service options available in the community to address their needs.
  • Information and referral function to appropriate service within CMHA-WECB or the community for mental health and addiction
  • Education
  • Assessment services provide timely comprehensive medical psychological and social assessments for individual experiencing mental health issues and requiring access to services. Individuals may then be linked to a wide variety of services to support people and their families within CMHA-WECB or the community.
  • Crisis planning/intervention/prevention
  • Person centered planning
  • Supportive counselling
  • Advocacy

Admissibilité / population desservie: People who may or may not be diagnosed with a mental illness who need information, assessment, and potentially intensive case management * people who need follow-up care from acute inpatient services from hospital * people who might not be eligible for intensive case management but need help to ensure they are appropriately linked to community services
