|
Complex continuing care unit providing specialized services to a priority population of those who have major neurocognitive impairment or cognitive deficits/impairments with associated responsive behaviours. These patients are cared for over an extended period of time to maintain and slow the rate of function. Patient’s care needs are not able to be met in the community.

Program eligibility assessed and determined by Unit Charge Nurse in consultation with admitting physician based on admission criteria. Internal Bluewater Health referrals are initiated within the Health Information System. For external referrals call Patient Flow at 519-464-4400 ext. 5355 between the hours of 8:00AM to 4:00PM.
